About Dukoral — Oral Cholera & Traveller's Diarrhea Vaccine

Dukoral is an oral, inactivated vaccine that provides dual protection against two major causes of serious gastrointestinal illness during international travel. It contains killed Vibrio cholerae O1 bacteria and the recombinant cholera toxin B subunit, which cross-reacts with ETEC heat-labile toxin (LT) to provide partial protection against traveller's diarrhea. Cholera causes rapid, severe dehydration that can be fatal within hours without treatment — particularly dangerous in resource-limited settings.

Dukoral provides approximately 85% protection against cholera and approximately 60% protection against ETEC-related traveller's diarrhea. Adults require 2 oral doses taken 1–6 weeks apart, with the final dose completed at least 1 week before arrival in an endemic area. A booster is needed every 2 years for cholera protection and every 3 months for ETEC diarrhea protection if risk continues. Travel to Cholera-Endemic Africa

Sub-Saharan Africa, including parts of the Democratic Republic of Congo, Ethiopia, Kenya, Nigeria, and Somalia, has active cholera transmission. Travellers to these regions benefit most from Dukoral.

However, Dukoral does NOT protect against traveller's diarrhea from other causes, including Campylobacter, Salmonella, Shigella, norovirus, or non-LT ETEC strains. For this reason, Dukoral should be considered a supplement to safe food and water practices, not a replacement. It is most beneficial for high-risk travellers heading to areas with poor sanitation.
How is Dukoral taken?
Dukoral is an oral vaccine — not an injection. Each dose is dissolved in 150 mL of clean, cool water (a sachet of effervescent buffer is included) and consumed orally. Adults require 2 doses taken 1–6 weeks apart. For full protection, the last dose should be completed at least 1 week before entering an endemic area. Food and drink should be avoided for 1 hour before and after each dose. The drink has a slightly chalky taste. How far in advance do I need to take Dukoral before travel?
Ideally, you should begin the Dukoral series at least 2 weeks before departure so both doses are complete and 1 week of post-dose immunity establishment time has passed. The two doses must be taken 1–6 weeks apart — taking them more than 6 weeks apart may require restarting the series. For traveller's diarrhea (ETEC) protection, both doses must be completed before exposure. Who should avoid Dukoral?
Dukoral should not be given to individuals with a known severe allergy (anaphylaxis) to any component of the vaccine, including formaldehyde. It is not approved for children under 2 years of age. Dukoral contains trace residual formaldehyde from inactivation — this is not a concern for most people but should be disclosed if relevant. Immunocompromised individuals can generally receive Dukoral as it is an inactivated vaccine, but should discuss with their healthcare provider. Pregnancy is not a contraindication, though data is limited — discuss with your OB or travel health pharmacist.
Does Dukoral protect against all causes of traveller's diarrhea?
No. Dukoral protects specifically against cholera (V. cholerae O1) and ETEC strains that produce heat-labile toxin (LT).
