Why Hepatitis A Vaccination Matters for Travellers

Hepatitis A is a viral liver infection transmitted through contaminated food and water. It is common in regions with poor sanitation infrastructure — including popular travel destinations across South Asia, Southeast Asia, Africa, Central America, and the Caribbean. Even resort-style hotels are not immune, as contamination can occur at the source.

There is no treatment for hepatitis A — only supportive care. Infection can cause weeks of debilitating illness including jaundice, fatigue, abdominal pain, and nausea, and in rare cases can progress to acute liver failure. The vaccine is highly effective (95%+ after 2 doses) and provides decades of protection.

Do I need a hepatitis A vaccine before travelling?

Hepatitis A vaccination is recommended for all travellers visiting developing countries where hepatitis A is endemic — which includes most of Asia, Africa, Central and South America, and parts of Eastern Europe and the Middle East. Even short trips to resort destinations (Cancun, Dominican Republic, Jamaica) carry some risk. The vaccine is highly effective and provides decades of protection after two doses.

How quickly does the hepatitis A vaccine work?

The first dose of hepatitis A vaccine provides protection in approximately 2 weeks for most healthy adults. It is advisable to receive the vaccine at least 14 days before departure. Even for last-minute travellers, getting vaccinated the day of or shortly before travel is still beneficial — combined with immunoglobulin (Ig) for very high-risk situations.

Is hepatitis A vaccine covered by OHIP?

Hepatitis A vaccination is not covered by OHIP for travel purposes. However, hepatitis B vaccine (and by extension Twinrix, the Hep A+B combination) is publicly funded in Ontario for students in Grade 7 under the school immunization program. What is the difference between a hepatitis A vaccine and Twinrix?

Single-antigen hepatitis A vaccines (Havrix, Avaxim) protect against hepatitis A only, in a 2-dose series. Twinrix is a combination vaccine that protects against both hepatitis A and hepatitis B, given in a 3-dose series. If your travel health assessment indicates you need both vaccines — which is common for travel to Asia, Africa, or any destination with healthcare exposure risk — Twinrix is more convenient and often more cost-effective.

Can I get the hepatitis A vaccine if I have already had hepatitis A?

People who have previously had hepatitis A infection develop lifelong natural immunity and do not need the vaccine. If you are unsure whether you have had hepatitis A, a blood test (serology) can check for antibodies. For most travellers, it is simpler and less expensive to just vaccinate rather than test first.

What are the side effects of the hepatitis A vaccine?

The hepatitis A vaccine is well tolerated. Common side effects include soreness at the injection site (most common), mild headache, and fatigue for 1–2 days. Serious allergic reactions are very rare. The vaccine does not contain live virus and cannot cause hepatitis A infection.
