Pneumococcal disease — caused by the bacterium Streptococcus pneumoniae — is responsible for pneumonia, meningitis, and bacteraemia (bloodstream infection). In older adults and those with chronic conditions, pneumococcal pneumonia can be severe and life-threatening. Vaccination is the most effective prevention strategy. Ontario publicly funds pneumococcal vaccination for adults 65 and older, and Peel Best Pharmacy offers walk-in vaccination without an appointment.

Who Should Get the Pneumonia Vaccine?

Adults 65 and Older

Pneumococcal disease causes significant morbidity and mortality in older adults. Ontario publicly funds pneumococcal vaccination for all adults 65+. If you have not yet received it, walk in today.

Adults with Chronic Lung Disease

COPD, asthma, and other chronic respiratory conditions significantly increase the risk of severe pneumococcal pneumonia. Pneumococcal vaccination is recommended regardless of age.

Adults with Diabetes

Diabetes impairs immune function and increases susceptibility to invasive pneumococcal disease. NACI recommends pneumococcal vaccination for all adults with diabetes mellitus.

Adults with Heart or Liver Disease

Chronic heart failure, cirrhosis, and chronic liver disease are NACI-listed indications for pneumococcal vaccination.

Immunocompromised Individuals

Patients on immunosuppressive therapy, those with HIV, asplenia, or receiving chemotherapy have increased risk of serious pneumococcal infection and may require a specific vaccination sequence.

Current or Former Smokers

Smoking damages the respiratory tract and significantly increases susceptibility to pneumococcal pneumonia. Vaccination is recommended for current smokers and those with smoking-related lung damage.

Is the pneumonia vaccine free in Ontario?
Ontario publicly funds pneumococcal vaccination for adults 65 and older. As of the updated Ontario Publicly Funded Immunization Schedule, PCV20 (Prevnar 20) is the recommended vaccine for seniors in this age group. Certain high-risk individuals under 65 — including those with immunocompromising conditions, asplenia, or serious chronic diseases — may also be eligible for publicly funded vaccine. For patients not covered by public funding, private insurance often covers pneumococcal vaccines. Our pharmacist can review your eligibility.
What is the difference between Prevnar 20 and Pneumovax 23?
Prevnar 20 (PCV20) is a conjugate vaccine that generates a stronger and more durable immune response and is now the preferred vaccine for most adult patients, including seniors. It covers 20 strains of Streptococcus pneumoniae. Pneumovax 23 (PPSV23) is an older polysaccharide vaccine that covers 23 strains but produces a less durable antibody response and cannot be boosted effectively with repeat doses. NACI's current guidance for most adults 65+ is a single dose of PCV20. For high-risk immunocompromised individuals, a sequence including both vaccines may be recommended. Do I need a prescription for the pneumonia vaccine?
In Ontario, pharmacists are authorised to administer pneumococcal vaccines without a physician's prescription for adults who meet eligibility criteria. You can walk in to Peel Best Pharmacy, and our pharmacist will screen you for eligibility and any contraindications before administering the vaccine.
Can I get the pneumonia vaccine and the flu shot on the same day?
Yes. NACI guidance supports co-administration of pneumococcal vaccines with other vaccines, including influenza and COVID-19 vaccines. There is no required interval between them. Combining your pneumonia vaccine, flu shot, and COVID booster in a single visit is efficient and does not reduce effectiveness.
How long does the pneumonia vaccine last?
Prevnar 20 (PCV20) provides durable protection through a conjugate immune response. Most adults 65+ will require only a single dose of PCV20 in their lifetime. Pneumovax 23 (PPSV23) provides 3–5 years of protection for most patients but cannot be repeated as a true booster (repeat doses do not reliably increase antibody levels). For most adults, the current guidance results in a one-time vaccination with PCV20. Patients at very high risk may be given a different schedule.
I already received the older Prevnar 13. Do I need Prevnar 20?
Prevnar 20 (PCV20) covers the 13 strains in Prevnar 13 plus 7 additional strains. NACI recommends that adults who have previously received PCV13 but not PCV20 receive a dose of PCV20, typically with a minimum interval of 1 year after the previous pneumococcal conjugate vaccine. If you previously received only PPSV23 (Pneumovax 23), a dose of PCV20 is recommended with appropriate timing.
