MedsCheck is a free annual medication review program funded by the Ontario government for patients who take three or more chronic medications. At your MedsCheck appointment, your pharmacist conducts a private, one-on-one review of every medication you take — prescription drugs, over-the-counter medications, vitamins, and supplements. The pharmacist checks for potential drug interactions, duplicate therapies, incorrect dosing, and medications that may no longer be necessary. You receive a printed medication record and action plan. MedsCheck is also available for residents of long-term care facilities (MedsCheck Long-Term Care) and patients recently discharged from hospital (MedsCheck at Home). Is MedsCheck covered by OHIP?
Yes. MedsCheck is fully covered by the Ontario government for eligible residents who take three or more chronic medications. There is no cost to you.
Who is eligible for MedsCheck?
Any Ontario resident who takes three or more chronic medications is eligible for an annual MedsCheck. This includes patients with diabetes, heart disease, high blood pressure, asthma, and other ongoing conditions.
How long does a MedsCheck appointment take?
Most MedsCheck appointments take 20–40 minutes. You will receive a full medication review, a printed medication record, and personalized recommendations.
What should I bring to my MedsCheck appointment?
Bring all your medications — prescription bottles, OTC products, vitamins, and supplements. The more complete your list, the more thorough the review.
Can I get a MedsCheck if I use a blister pack?
Yes. If you are already on a blister pack program with us, your MedsCheck will be completed as part of your regular care. Call (905) 712-0003 to schedule.
