Immigration Medical Examination — What It Is and Why It's Required

The IRCC immigration medical examination is a mandatory health assessment for most permanent residence applicants and certain temporary visa categories. Canada uses the medical to assess whether an applicant has conditions that could pose a danger to public health, a danger to public safety, or would place an excessive demand on Canada's health or social services.

The examination must be conducted by a panel physician — a physician designated and authorized by IRCC. Peel Travel Clinic and Saldanha Health Centre has a panel physician on staff who performs these examinations and submits results directly to IRCC through the eMedical portal. An appointment is required.

Who Needs an Immigration Medical Examination?

Most permanent residence applicants and some temporary visa holders are required by IRCC to complete an immigration medical.

Permanent Residence applicants

All categories — family class, skilled worker, investor, provincial nominee, refugee, etc.

Certain temporary visa applicants

Long-term work permits, study permits (>6 months in some countries), caregiver programs.

Refugee claimants

Required as part of the refugee determination process.

Renewal / update applicants

Some applicants need a new medical if their previous exam has expired (typically valid 12 months) before a final decision is made.

What Is Included in the Examination?

The immigration medical exam is a comprehensive multi-component assessment. Components required vary by age and country of origin.

Physical Examination

General physical exam including height, weight, blood pressure, vision, and assessment of organ systems. The physician looks for conditions that may be grounds for medical inadmissibility under IRCC guidelines.

Blood Tests (Serology)

Syphilis serology (RPR/VDRL) required for all applicants 15 and older. HIV testing required for applicants from certain countries or who meet specific criteria. Hepatitis B serology for certain applicants.

Chest X-Ray

Required for applicants aged 11 and older. Screens for tuberculosis (TB) and other pulmonary conditions. Additional TB testing (IGRA/TST) may be required if the X-ray is abnormal or risk factors are present.

Vision Assessment

Basic visual acuity test. Colour vision and other assessments if clinically indicated.

Medical History Review

Review of past and current medical conditions, surgeries, medications, mental health history, and substance use. Honest disclosure is essential — providing false information is grounds for inadmissibility.

Immunization Review

Review of vaccination status. Children may require documentation of childhood immunizations. Some applicants may be asked about specific vaccines relevant to country of origin.

What to Bring to Your Immigration Medical Appointment

Valid passport or travel document

All biographical pages must be visible. The panel physician records your passport details as part of the exam.

Vaccination records

Any childhood immunization records, yellow fever certificate, or other vaccine documentation you have. Not mandatory but helpful.

List of current medications

Include dosages. Especially important for psychiatric medications, immunosuppressants, and HIV/hepatitis medications.

Relevant medical records

If you have been treated for tuberculosis, hepatitis B/C, HIV, or significant psychiatric conditions, bring relevant treatment records if available.

IRCC instruction letter (if provided)

Some applicants receive specific IRCC instructions about their medical exam. Bring this letter if you have it.

Payment

Immigration medicals are not covered by OHIP. Payment is due at the time of service. Call ahead for current fee details.

What is an IRCC immigration medical exam?
The Immigration, Refugees and Citizenship Canada (IRCC) immigration medical examination is a mandatory health assessment required for most permanent residence applications and some temporary visa categories. It must be conducted by an IRCC-designated panel physician — a physician who has been specially authorized by IRCC to perform immigration medicals and submit results directly to IRCC through the eMedical portal. How long is the immigration medical exam valid?
The immigration medical examination is valid for 12 months from the date of completion. If IRCC does not make a final decision on your application within that 12-month window, you may be asked to undergo a new medical examination. You can check your IRCC portal to confirm the expiry date of your medical results.
What happens if a medical condition is found during the exam?
If the panel physician identifies a condition, they report it to IRCC using standardized codes. Not all medical conditions lead to inadmissibility — IRCC assesses whether the condition poses an excessive demand on health or social services in Canada, or is a danger to public health or safety. Some conditions that previously made applicants inadmissible have been updated in IRCC policy. A medical finding does not automatically mean a refusal — applicants have the right to respond to a procedural fairness letter if inadmissibility is being considered.
What do I bring to my immigration medical exam?
Bring: your valid passport or travel document (with all pages intact), any previous vaccination records you have, a list of current medications, reading glasses if you use them for vision correction, any medical records relevant to chronic conditions (especially if you have been previously treated for tuberculosis, hepatitis B, or other significant conditions), your IRCC instruction letter if provided, and a valid form of payment (immigration medicals are not covered by OHIP). Children must be accompanied by a parent or guardian.
Can I get travel vaccines at the same visit as my immigration medical?
Yes. Peel Travel Clinic is both an immigration medical examination site and a full travel vaccine clinic. If you are travelling internationally as part of your immigration process — or simply wish to get travel vaccines at the same visit — our team can administer required travel vaccines on the same day as your immigration medical. Is the immigration medical exam covered by OHIP?
No. Immigration medical examinations are not covered by OHIP or any provincial health plan. They are privately paid services. Fees cover the panel physician examination, required laboratory tests, chest X-ray, and submission to IRCC through the eMedical system. Call (905) 712-0003 for current fee information. How do results get sent to IRCC?
Panel physicians submit examination results directly to IRCC through the eMedical portal — an encrypted electronic system. You do not need to send any documents yourself. Once submitted, results are linked to your IRCC application file automatically. You can confirm receipt by checking your IRCC online account or MyCIC portal. Results are typically submitted within a few business days of completing all components of the exam.
