What Is MedsCheck?
MedsCheck is a provincial program funded by the Ontario Drug Benefit (ODB) program that provides free, structured medication reviews conducted by a licensed pharmacist. The program was designed to address a well-documented problem: many Ontarians — particularly seniors, people with multiple chronic conditions, and those seeing multiple specialists — take a complex combination of medications without ever having those medications reviewed together as a whole picture.
Studies show that medication-related problems — including adverse drug reactions, drug interactions, inappropriate dosing, and non-adherence — account for a significant portion of hospital admissions and emergency department visits, especially in older adults. MedsCheck aims to catch these problems before they cause harm.
During a MedsCheck, your pharmacist reviews every medication you take — prescription drugs, over-the-counter medications, vitamins, and herbal supplements — and creates a personalized medication record. The review identifies potential drug interactions, duplications (two drugs doing the same thing), medications that may be inappropriate for your age or kidney function, and adherence issues. You receive a printed Medication Record to share with your doctor.
Types of MedsCheck Available in Ontario
Ontario offers four types of MedsCheck services. The right type depends on your medical situation, living situation, and frequency of need.
MedsCheck Annual
A comprehensive review of all your medications — prescription, OTC, and supplements — to identify interactions, duplication, and adherence issues.
MedsCheck for Diabetes
Focused review of diabetes medications, blood glucose monitoring practices, A1C targets, and lifestyle factors. May include diabetes education.
MedsCheck at Home
Pharmacist travels to the patient's home to conduct the medication review in a familiar setting. Includes family/caregiver participation.
MedsCheck Follow-Up
A shorter 20–30 minute check-in to review changes made since the last MedsCheck, monitor adherence, and address new concerns.
What Happens During a MedsCheck at Peel Best Pharmacy?

Schedule your appointment
Call us at (905) 712-0003 or walk in to book a dedicated 45–60 minute appointment with one of our pharmacists. Bring all your medication bottles, OTC products, vitamins, and supplements.

Comprehensive medication review
Our pharmacist reviews every medication you take — what it is for, how you take it, how long you have been on it, and any concerns you have. We assess for interactions, duplications, inappropriate dosing, and adherence issues.

Your personalized Medication Record
We create a printed Medication Record listing all your medications with plain-language descriptions — what each drug is, why you take it, and how to take it correctly. This document is yours to keep and share with your doctors.

Action plan and referrals
If we identify concerns — for example, a potential drug interaction, a medication that is no longer appropriate, or a dose that should be adjusted — we document these and communicate with your prescriber on your behalf with your consent.

Follow-up MedsCheck if needed
If significant changes were made, we may recommend a MedsCheck Follow-Up (covered by ODB) within the year to review how the changes are working and address any new questions.
